Photography Booking Form Template

A split-screen booking for a portrait photographer: contact details, the kind of shoot, a preferred date, studio or on location, and ideas for the mood, then a choice of three packages and an optional rush edit, paid on the form.

What a photography booking form is for

A photography booking form takes a session request and the payment together, so a date is only held for a client who has paid. It also collects what the photographer needs to plan the shoot: the kind of session, the place and the ideas behind it.

This template asks the planning questions first and then moves to a choice of three packages, Mini, Classic and Full day, with Classic marked “Most booked”. The location answer carries its own price, so shooting on location adds a travel charge automatically, and rush editing is offered as an add-on beside the pay button. The thank-you screen uses the Picture look.

Who uses it

  • Portrait, family and headshot photographers taking paid bookings.
  • Wedding and event photographers taking a booking deposit.
  • Videographers, makeup artists and stylists who sell packages.
  • Studios renting time by the hour or the day.

How to customise it

A template is a starting point: every word, question and colour stays editable.

  • Your packages

Rename the three packages and set your prices, up to ten per form; move the “Most booked” badge to the package you want to stand out.

  • Let clients pick a time

Add a “Book a time” block that sends clients to your Vonza booking page to choose a slot, alongside the preferred date.

  • Take a deposit

Use a Flexible Payment to take part of the fee now and the rest later, on eligible plans, or an Installment Plan for bigger packages.

  • Add a model release

Add a Signature block or an Agreement & e-sign block so clients agree to how the photos may be used when they book.

What happens after someone submits

The same Vonza Forms settings run behind every template. These are the ones this form uses.

  1. 01

    The response arrives in Responses

Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.

  2. 02

You and your team are told

The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.

  3. 03

The respondent becomes a CRM contact

Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.

  4. 04

    They get a confirmation

Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.

  5. 05

    The payment is recorded

The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment.

  6. 06

    Automations and webhooks take over

Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.

Each paid booking appears in the Sales tab, and the client gets a receipt by email; the thank-you screen tells them when you will confirm the time.
