What a meal order form is for

A meal order form lets customers build an order from a menu, choose when it arrives and pay in one go. For meal-prep and catering businesses it replaces order messages that have to be totted up by hand.

This template prices every meal per portion with a Quantity block, so the total grows as the customer adds meals, from zero to fourteen of each. The second step takes the delivery slot and a full address, plus a phone number for the driver. At payment, a chilled delivery fee is added, a cold-pressed juice pack is offered next to the pay button, and a code box takes discount codes. The thank-you screen uses the Recap look, so the customer sees what they ordered.

  • Meal-prep companies taking weekly orders with delivery.
  • Caterers, home cooks and dark kitchens selling a set menu.
  • Restaurants taking pre-orders for collection or a set delivery day.

How to customise it

A template is a starting point: every word, question and colour stays editable.

  • Your menu and prices

Rename each Quantity block to a dish and set its unit price and maximum, or switch to a Product list with up to thirty products.

  • Delivery or collection

Change the extra fee to always added or buyer chooses, rename it, or add a collection option and show the address only for deliveries.

  • Sell a weekly plan

Add a Subscription Plan price, billed weekly, monthly or yearly, for customers who want the same box every week.

  • Close orders on cut-off day

Schedule the form to open on Monday and close before you shop for the week, with a message for anyone who arrives early.

What happens after someone submits

The same Vonza Forms settings run behind every template. These are the ones this form uses.

  1. 01

    The response arrives in Responses

Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.

  2. 02

You and your team are told

The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.

  3. 03

The respondent becomes a CRM contact

Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.

  4. 04

    They get a confirmation

Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.

  5. 05

    The payment is recorded

The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment.

  6. 06

    Automations and webhooks take over

Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.

Paid orders appear in the Sales tab with the buyer, items, amount and status; export them to CSV for the kitchen and the driver.
