Donation Form Template
A warm split-screen donation form for a local food bank: the donor’s name and email, an optional dedication in someone’s honour, an opt-in for updates, then a choice to give once, from suggested amounts or any amount, or to give monthly.

What a donation form is for
A donation form lets supporters give online in a minute: they choose an amount, say whether it is a one-off gift or a monthly one, and pay. For a charity or a cause it also collects the donor’s details, so every gift can be thanked and receipted.
This template asks for very little before the money: a name, an email for the receipt, an optional dedication and an optional updates opt-in. The submit button reads “Choose an amount” and leads to two choices. Give once uses the Pay What You Want price type with suggested amounts and a minimum, and Give monthly is a subscription marked “Biggest impact”. The code box is switched off, as it should be on a donation, and the thank-you screen uses the Celebration look.

How to customise it
A template is a starting point: every word, question and colour stays editable.
Your suggested amounts
Set your own suggested amounts, a minimum and a maximum on the Pay What You Want price, and your own monthly amount.
Say what a gift does
Rewrite the description line to tie an amount to an outcome, as the template does with a week of groceries.
Tribute gifts
Keep the dedication question, or add a yes or no for sending a card, with logic that shows an address only when needed.
Thank donors by name
The thank-you screen recalls the donor’s first name; the email to the respondent can thank them again in your own words.
What happens after someone submits
The same Vonza Forms settings run behind every template. These are the ones this form uses.
- 01
The response arrives in Responses
Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.
- 02
You and your team are told
The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.
- 03
The respondent becomes a CRM contact
Everyone who leaves an email (here, “Email for your receipt”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.
- 04
They get a confirmation
Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.
- 05
The payment is recorded
The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment.
- 06
Automations and webhooks take over
Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.
Monthly donors can be paused, resumed or stopped from their response when they ask (not for Paystack payments), and every gift is listed in the Sales tab with its status.
