What an activity waiver form is for
An activity waiver form, also called a release form, asks participants to confirm they understand the risks of an activity, declare anything that affects their safety and give an emergency contact before they take part. Online, it is signed before the visit instead of at a crowded front desk.
This template was written for a climbing gym and takes about two minutes. The health question is a yes or no; only a yes opens a box for details, through a logic rule. The emergency contact sits side by side with their phone number, and the form ends with a required risk statement and a drawn signature. The submit button reads “Sign and submit”.
Who uses it

How to customise it
A template is a starting point: every word, question and colour stays editable.
Your own waiver text
Rewrite the risk statement for your activity, or replace it with an “Agreement & e-sign” block that shows your full waiver, possibly asking people to scroll to the end before they agree and sign.
Minors and guardians
Add a question about the participant’s age and show a guardian’s name and signature with logic only for under-18s.
Sign once, not every visit
Allow one response per person by email address, so regulars are not asked to sign again.
Have the wording checked
Vonza collects the signature on the form; whether a waiver protects you depends on its wording and your local law, so have it reviewed.
What happens after someone submits
The same Vonza Forms settings run behind every template. These are the ones this form uses.
- 01
The response arrives in Responses
Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.
- 02
You and your team are told
The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.
- 03
The respondent becomes a CRM contact
Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.
- 04
They get a confirmation
Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.
- 05
Automations and webhooks take over
Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.
Search Responses by name at the front desk to find a participant’s signed form, or download it as a PDF to keep with your records.
To pair a waiver with a booking or entry, read about Vonza registration forms, or see the Vonza form builder and intake forms with consent and signatures.
