# Quote Request Form Template > A crisp quote request for local services, on one screen: which service, how big the job is, a description, photos of the job, a preferred date and the postcode, then the customer’s name and phone number. A Contact & leads template in the new Vonza Forms, “Service quote”: A crisp quote request for local services, with photos of the job. ## What a quote request form is for A quote request form collects what a service business needs to price a job without a site visit: what needs doing, how big it is, where it is, when the customer wants it and, ideally, photos. The more complete the request, the faster and more accurate the quote. This template is built for trades and home services. The service is a dropdown, the job size is a one-tap choice, and up to five photos of the job can be uploaded from a phone. A postcode tells you whether the job is in your area, and the form asks for a phone number rather than an email because the thank-you promises a price by text. ### Who uses it - Cleaners, painters, plumbers, electricians and gardeners pricing local jobs. - Removals, handyman and repair businesses that quote from photos. - Event suppliers, caterers and photographers who quote per booking. - Agencies and consultants who send a proposal after an enquiry. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Request a quote”. ### Questions - **Service**: Dropdown · required · Cleaning, Painting, Plumbing, Electrical, Garden - **Size of the job**: Multiple choice · Small, Medium, Large - **Tell us about the job**: Long answer - **Photos of the job**: File upload · Images, up to 5 files of 10 MB - **Preferred date**: Date · No past dates - **Postcode**: Short answer · required - **Name**: Name · required - **Phone**: Phone · required ### Button and thank-you screen - Submit button: “Get my quote” - Thank-you screen: “Quote request sent”, We’ll text you a price within a few hours. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Add an email question**: Add an Email block if you want the request in your CRM and an automatic confirmation; the template asks for a phone number only. - **Estimate a price as they go**: Give choices a price and add a Calculated block that works out an estimate from the answers. - **Ask follow-ups per service**: Add questions for each service, such as the number of rooms for cleaning, and show them with logic only when that service is picked. - **Your service area**: Swap the postcode for an Address block with street, city, postcode and country, or keep the postcode and add a note about where you work. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **Add an email to reach them later**: This template asks for no email, so nobody is added to your CRM. Add an Email question and every respondent who leaves one becomes a contact and can get a confirmation with their answers as a PDF. 4. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Use the email to you, sent to you and up to five teammates, to get each request the moment it arrives, so whoever is on call can price it straight away. Add a Calculated block that works out a number or money value from the answers, for example size of the job times a rate, and recall it in the form’s text. ### Can I embed the quote form on my website? Yes. Embed it inline, as a popup or slider, or behind a floating “Get a quote” button that opens on a click, after a delay or on exit intent.
