# Product Feedback Survey Template > A soft-gradient survey in two short pages for software product research: how often people use the product and which features matter most, then how satisfied they are, the one thing they would change and an email to join your research panel. A Surveys & feedback template in the new Vonza Forms, “Product feedback survey”: Soft gradient, two short pages for SaaS product research. ## What a product feedback survey is for A product feedback survey asks the people who use a product how they use it, what they value and what frustrates them. Product teams use the answers to decide what to build next and to find users willing to talk in more depth. This template splits the survey into usage and feelings, two short pages with a progress bar. Frequency comes first, because it frames every other answer: a daily user and an occasional one want different things. Features are checkboxes, satisfaction is a 1 to 5 scale, and the open question asks for one change only. The email is optional and framed as joining a research panel, so only volunteers leave it. ### Who uses it - SaaS product managers running quarterly product research. - Founders looking for early users to interview. - App and tool makers deciding which feature to build next. - Course and community creators asking members what to add. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Help shape Pulse”. Progress shows as progress bar. ### Page 1: How you use Pulse - **How often do you use Pulse?**: Multiple choice · required · Every day, A few times a week, A few times a month, Rarely - **Which features matter most to you?**: Checkboxes · Dashboards, Alerts, Reports, Integrations, Team spaces, Mobile app ### Page 2: How it feels - **Overall, how satisfied are you?**: Linear scale · required · 1 (Not at all) to 5 (Very) - **If you could change one thing, what would it be?**: Long answer - **Email (to join our research panel)**: Email ### Button and thank-you screen - Submit button: “Send” - Thank-you screen: “Thank you for helping”, Every answer is read by the product team. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Your product and features**: Rename the product and replace the feature list with your own; a Ranking block can ask people to put features in order instead. - **Segment by plan or account**: Add Hidden fields filled from the link, such as plan or account ID, so every answer arrives with the context you already know. - **Follow up the unhappy ones**: Show an extra question with logic when satisfaction is low, or end the survey early on a different thank-you screen. - **Run it inside your app**: Embed it as a slider or popup that opens after a delay, halfway down a page or on exit, instead of sending a link. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email (to join our research panel)”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Respondents who join the research panel become CRM contacts; tag them “research panel” so you can invite them to interviews later. How often people use the product, which features matter, how satisfied they are and what one thing they would change. This template asks those five questions over two pages. ### Can I see where people stop answering? Yes. The Analytics page shows where people leave, page by page, along with views, starts, completion and average time. ### Can I tie answers to a user without asking for their email? Yes. Add a Hidden field and pass a value in the survey link, such as a user or account ID; it is saved with the response and not shown. ### Can I translate the survey for users in other countries? Yes. A form can offer up to 10 of 30 languages, and Vonza AI can translate it for you using AI credits. Each language has its own link.
