# Photography Booking Form Template > A split-screen booking for a portrait photographer: contact details, the kind of shoot, a preferred date, studio or on location, and ideas for the mood, then a choice of three packages and an optional rush edit, paid on the form. A Orders & payments template in the new Vonza Forms, “Photo session booking”: Book and pay for a portrait session: three packages, a date and a rush-edit add-on. ## What a photography booking form is for A photography booking form takes a session request and the payment together, so a date is only held for a client who has paid. It also collects what the photographer needs to plan the shoot: the kind of session, the place and the ideas behind it. This template asks the planning questions first and then moves to a choice of three packages, Mini, Classic and Full day, with Classic marked “Most booked”. The location answer carries its own price, so shooting on location adds a travel charge automatically, and rush editing is offered as an add-on beside the pay button. The thank-you screen uses the Picture look. ### Who uses it - Portrait, family and headshot photographers taking paid bookings. - Wedding and event photographers taking a booking deposit. - Videographers, makeup artists and stylists who sell packages. - Studios renting time by the hour or the day. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Book a portrait session with Nadia Rahman”. ### Questions Studio or on location in the city. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **The payment is recorded**: The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment. 6. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Each paid booking appears in the Sales tab, and the client gets a receipt by email; the thank-you screen tells them when you will confirm the time. Yes. The template ends with three packages and a “Book and pay” button; clients pay by card, Apple Pay, Google Pay or PayPal before the booking is complete. ### How does the travel charge work? Add up to five add-ons next to the pay button, each with a description, an image, a badge and a was-price if you want one. ### Can clients choose an exact time slot? Add a Book a time block that links to your Vonza booking page, where clients pick a slot from your availability.
