# Donation Form Template > A warm split-screen donation form for a local food bank: the donor’s name and email, an optional dedication in someone’s honour, an opt-in for updates, then a choice to give once, from suggested amounts or any amount, or to give monthly. A Donations template in the new Vonza Forms, “Food bank donation”: Give once or monthly with suggested amounts and an optional dedication. ## What a donation form is for A donation form lets supporters give online in a minute: they choose an amount, say whether it is a one-off gift or a monthly one, and pay. For a charity or a cause it also collects the donor’s details, so every gift can be thanked and receipted. This template asks for very little before the money: a name, an email for the receipt, an optional dedication and an optional updates opt-in. The submit button reads “Choose an amount” and leads to two choices. Give once uses the Pay What You Want price type with suggested amounts and a minimum, and Give monthly is a subscription marked “Biggest impact”. The code box is switched off, as it should be on a donation, and the thank-you screen uses the Celebration look. ### Who uses it - Food banks, shelters and local charities taking online gifts. - Churches and faith groups collecting tithes and offerings. - Schools, clubs and parent associations fundraising for a project. - Creators and podcasters who accept support from their audience. ## What’s in the template Every question exactly as the template builds it, page by page. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email for your receipt”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **The payment is recorded**: The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment. 6. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Monthly donors can be paused, resumed or stopped from their response when they ask (not for Paystack payments), and every gift is listed in the Sales tab with its status.
