# Client Intake Form Template > A warm, two-step intake for consultants and agencies: first who the client is, then what they want to achieve, the budget, the ideal launch date and any brief or reference files. A Applications & intake template in the new Vonza Forms, “Client intake”: Warm, editorial two-step intake for consultants and agencies. ## What a client intake form is for A client intake form gathers what you need to know about a new client before the first call: who they are, what they are trying to achieve, what they can spend, when they need it and anything they have already written down. Done well, it turns a vague enquiry into a brief you can price. This template splits the questions into two short steps so the client starts with the easy part. The project step asks one open question about goals, a budget band rather than a number, a launch date that cannot be in the past and an upload for a brief or reference files. Progress shows as steps, and the thank-you screen promises a call within two working days, which you can change to your own timing. ### Who uses it - Marketing, design and development agencies qualifying new projects. - Consultants and freelancers who want a brief before the discovery call. - Accountants, lawyers and other professional services onboarding a client. - Coaches who sell packages and need goals and timing up front. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “New client intake”. Progress shows as steps. ### Page 1: About you - **Name**: Name · required - **Email**: Email · required - **Phone**: Phone - **Company**: Short answer - **Website**: Link ### Page 2: Your project - **What are you hoping to achieve?**: Long answer · required - **Budget**: Dropdown · Under $2k, $2k – $10k, $10k – $30k, $30k+ - **Ideal launch date**: Date · No past dates - **Brief or reference files**: File upload · Documents, up to 3 files of 25 MB ### Button and thank-you screen - Submit button: “Send intake” - Thank-you screen: “Thanks, {{name.first}}”, We’ll review and book a call within two working days. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Book the call on the spot**: Add a “Book a time” block, which sends the client to your Vonza booking page, so the intake ends with a call in the diary instead of an email thread. - **Your own budget bands**: Edit the dropdown options to the ranges you actually quote, or swap the dropdown for a Slider if you prefer a number. - **Ask follow-ups by service**: Add a question about the service, then use logic to show only the questions that service needs and skip the rest. - **Let them finish later**: Turn on “Let people continue later” for longer intakes: the client gets a private link to finish within 30 days. Files are not kept on a saved draft. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Give each intake a review status (New, In progress or Done) and add team notes only your team sees, so whoever takes the call knows what was already discussed.
